During the last ceremony we had a hard time with personnel that were inadvertently left out of some submitted incidents. We are a group of volunteers that cannot verify each person involved in an incident. We must make decisions based on the nominations and names submitted on the form. Please make sure that when you submit a nomination you do not miss anyone. Once we have plaques made we cannot have them remade to include a name that was left out, although we can award a medal or ribbon afterward. FYI, our future plans include getting a google drive so the forms can be submitted electronically. Thanks!
